What it is

Sildenafil is a prescription medicine for erectile dysfunction and the active ingredient inside Viagra. It has been prescribed since 1998, which means its effects, its limits, its interactions and its failure modes are documented about as thoroughly as any drug in circulation. There is nothing experimental about it.

The mechanism, without jargon

Arousal causes your body to release a chemical messenger that tells the arteries feeding the penis to relax and widen. Blood arrives faster than it can leave, pressure builds inside two spongy chambers, and that pressure pinches shut the veins that would otherwise drain them. That is an erection, and it is a hydraulic event rather than a muscular one.

An enzyme called PDE5 exists to clear that messenger away, which is how an erection ends. Sildenafil occupies the enzyme and slows it down. Nothing else about the process changes. The messenger survives longer, the arteries open further, and a normal amount of arousal travels considerably further than it otherwise would.

The point men most often miss

It protects a signal your body produces. It does not produce one. No arousal means there is nothing to protect, which is behind almost every report of a dose that "did nothing".

Side effects

  • Unstable angina, or chest pain brought on by sexual activity
  • Blood pressure that is very high or very low and not controlled
  • Severe liver or kidney impairment
  • Previous sudden vision loss from the eye condition NAION

Certain antifungals, some antibiotics, HIV protease inhibitors and grapefruit juice in quantity all change how quickly your body clears sildenafil, which changes the appropriate dose. That is why the health form asks for everything rather than just prescriptions.
